Required Onboarding Credentials

The items on this list must be provided during the application process. They can be uploaded directly into the application or emailed to a Staffing@staffreliefinc.com.

  • Copy of Current Clinical License

  • Copy of Current Professional Liability Insurance (If Available)

  • Copy of Current Driver’s License

  • Copy of Social Security Card

  • Copy of Passport (in Place of Driver’s License)

  • Copy of Current CPR Certification

  • Copy of Other Certifications (CNA, CNOR, TNCC, CORT, ACLS, CCNR, PALS, BTLS, CEN)

  • TB Test (Within Past 12 Months) or Chest X-Ray

  • Mumps, Measles, and Rubella Titer (or Proof of Two Doses of MMR)
