Position Summary:
We are seeking an experienced Registered Nurse for a 8-week Home Health / Hospice travel assignment. The RN will provide skilled nursing care in the home setting. This role involves comprehensive care planning, coordination with interdisciplinary teams, patient and caregiver education.
We are seeking an experienced Registered Nurse for a 8-week Home Health / Hospice travel assignment. The RN will provide skilled nursing care in the home setting. This role involves comprehensive care planning, coordination with interdisciplinary teams, patient and caregiver education.
You’ll manage a daily caseload of 4–6 patients (home health) or 4–5 patients (hospice), depending on assignment. Mileage is paid between patient homes.
Key Responsibilities:
- Perform patient assessments, develop individualized plans of care, and coordinate services with physicians, patients, families, and the care team.
- Provide direct skilled nursing interventions, including wound care, IV therapy, PICC line management, catheter care, and disease-specific education (e.g., CHF, COPD).
Requirements:
- Active RN license in Georgia (or multistate compact).
- Current BLS certification.
- Valid driver’s license and proof of vehicle insurance.
- Minimum 1 year recent experience in Medical/Surgical nursing and either Home Health or Hospice (ICU experience preferred).
- Proficiency with EMR systems (Epic and/or Home Care Home Base preferred).
