
4 hospitals picked to pilot American Heart Association home care program
The American Heart Association has partnered with health tech company Cadence to launch a home care program aimed at reducing 30-day readmissions for heart failure patients.
Participating hospitals can refer patients to the Connected Care program prior to discharge, a process the AHA will help integrate into existing discharge workflows. Cadence will handle program enrollment, patient education, remote patient monitoring and clinical support, according to an Aug. 26 news release from the organization.
“By combining advanced remote patient monitoring technology with […] guideline-directed care and chronic condition management, we can help extend the high-quality care hospitals provide, ensuring people with heart failure receive proactive, timely support at home when they need it most,” John Meiners, the American Heart Association’s chief of mission-aligned businesses, said in the release.
The program is in its pilot phase at four U.S. hospitals:
- Texas Health Resources Texas Health Allen
- Lifepoint Health Rutherford (N.C.) Regional Medical Center
- Lifepoint Health Frye Regional Medical Center (Hickory, N.C.)
- Montage Health Community Hospital of the Monterey (Calif.) Peninsula
